Safeguard Plus Membership Fee   

To complete your membership application, you need to pay your annual Iasis Safeguard Plus membership fee.

Please use the Iasis Shoppe to pay the agreed non-refundable review fee of $25.  

Once the fee is paid, the membership fee request will be forwarded for the Iasis Safeguard Plus confirmation.  You will receive your membership approval and number within 3-5 business days.  You will then be able to access all of the Iasis Safeguard and Iasis Safeguard Plus services.

You will need your Visa or MasterCard to use the Iasis Shoppe shopping cart.
